I bought these 2x4 pads for my wife who suffering a lower back pain from a sacro-iliac joint arthritic pain. She had been using a couple of 2x2 sized pads putting on each side,but switching into only one pair of the 2x4 pads (one on each side) and on either channel "A" or channel "B". This will produce more intensity that she needs to reduce the level of intensity to the lower level (compared to the one with the 2 x2 pads on both channels ). She loves it and does not want to go back to use the smaller size again.

Please see my reviews on the replacement lead wires and the lumbar pads. Like the lumbar pads this size was new to me when I logged on to order some of the regular sized pads. This particular size works so nicely in the upper back/shoulder area to cover a larger area than the regular smaller pads. Very helpful and having their TENS unit have the possibility of two lead wires I am able to use these pads for the upper back/shoulder blade and use the regular smaller pads on the shoulder joint creating the best of both worlds.
I love my new TENS unit and this size of pad is great for one section of my back that is the main pain culprit. Trumedic has excellent products that work fast, free shipping.

I like the convenience of the larger pad. I don't have to be as exact in my placement, plus I feel like I get a better range of coverage during the treatment.

The 2x4 pads are my new favorite. I have a lot of joint pain and have found the more beneficial than the smaller electrode pads. Keep in mind that you will need to turn up the strength however since there is more surface area. That said, I haven't found that I need the power higher than the unit allows. I feel these are going to get a lot of use in my home!
I bought these because the standard smaller electrodes that came with my device aren't big enough to adequately cover the areas of my back where I have pain. However, I neglected to consider that the output is still the same voltage, but over a larger surface area-- therefore, it only feels about half as powerful as using the smaller electrodes. This might be okay if you have mild/moderate muscle pain but for me it takes a whole lot to feel any difference. For maximum effect, I wouldn't recommend using both 2x4 pads on the same port, but use just one along with a smaller electrode placed on the muscle's trigger point. On a more positive note, they do work very well on the neck/shoulders because those are much more sensitive areas with less densely-packed muscle (in my case, at least). It can wrap around the shoulder or side of the neck to distribute the pulse evenly, rather than just concentrated in one small area-- especially good if its your entire upper trapezius that is tight. To summarize: Pros--can cover a larger surface area, so you don't have to spend forever searching for that "sweet spot" to place it. Cons--less intense; not recommended for difficult areas that require a really high setting.
I did heavy deadlifting. I am usually quite sore latter. I used the 2x4 pads on my hamstring muscles and felt terrific. Made a big difference this time.
